Student Forms
Each student has two forms that need to be completed in order to register for classes with Mr. D’s Driving School.
1) Download one of the Driver Education Contract and Requirement Forms that apply to you:
2) Download one of the Illinois Secretary of State Approval Forms that apply to you:
- If you are currently enrolled in a public or private school, State Approval Form.
- If you are currently home schooled, State Approval form.
- If you are out of school, but under age 18, State Approval Form.
